Insurance and Payment
Full payment for the service is due at the time of your visit unless other arrangements were made before your appointment.
If your insurance has out-of-network benefits, we are happy to submit a claim to your insurance company on your behalf. The reimbursement amount depends on your insurance plan.
For questions about your insurance benefits, please contact your insurance carrier's customer service department or your employer's HR department.
Most lab work can be done in our clinic and billed directly to your insurance. For specific coverage details, please contact your insurance provider.
Appointment Policies
To book an appointment, please call us or fill out our contact form online. To ensure a smooth visit on the day of the appointment, please keep the following in mind:
- Please arrive on time for your appointment.
- Please bring your insurance card if you would like us to submit your visit to your insurance provider.
New Patients
All new patients will be asked to complete our patient intake forms. When you make an appointment, you receive an invitation to our patient portal to complete them in advance, saving you time. Alternatively, you can print the forms at home and bring them with you to your appointment for convenience.
Cancellations
Other than for serious emergencies, we expect all patients to keep their appointments. If you need to reschedule, please call our office at least 24 hours in advance to arrange a makeup appointment. We understand that schedules can change, but the 24-hour notice allows us to accommodate other patients who may need medical attention.
Medical Records
You can request copies of your medical records, whether for personal use or a visit with another healthcare provider. To get copies of your records, please complete the medical records release authorization form. The completed form can be faxed, mailed, or dropped off at our office. Once we receive the request, we will process and provide your records in a timely manner.

The most efficient way to reach us is through our patient portal. We encourage you to use this method of communication for seamless and streamlined interactions with our office.
Patient Intake Form
To make your visit stress-free and smooth, please complete the patient intake form before your appointment. This helps us gather essential information and ensures a more efficient experience when you arrive.
Prescription Refills and Renewals
At each office visit, we typically authorize enough refills to cover you until your next appointment. Before contacting the office for a prescription refill, please check your existing prescription.
If your prescription has existing refills (this is clearly printed on the pharmacy label), please call the pharmacy - not the office - to request the refill. Otherwise, we are happy to authorize prescription refills if you are current with your care.
We strongly encourage you to request prescription refills before you run out of medication. While we make every effort to authorize refills as soon as possible, we ask that you allow us three business days to complete prescription authorizations.
More and more often, commercial insurance carriers are requiring us to complete prior authorizations for prescriptions. A prior authorization is not generated until we have submitted a prescription to a pharmacy. Prior authorizations usually take an additional 48-72 hours to be processed by us, and then by your insurance carrier.
Retail pharmacies do not always have medications in stock, which can delay the availability of your medication. In order to avoid delays in obtaining your prescriptions, please place requests for refills at least two weeks before you expect to run out of medication.
Here are some tips to help ensure fast and affordable refills:
- Look for discount coupons on the product’s website.
- Check for third-party coupons online or on your phone. Apps such as GoodRx, SingleCare, and RxSaver may offer prices lower than your prescription coverage copays.
- Your medication may be available at a lower price through mail-order pharmacies, such as Amazon or Cost Plus Drugs.
- Commercial insurance companies contract with mail-order pharmacies, such as Caremark and Express Scripts. Contact your insurance company's customer service department or your employer's HR department to find out if you might be able to obtain your prescriptions through mail-order pharmacies. Often, prescription costs are lower at these pharmacies, and, for added convenience, your medications are delivered directly to your door.
