During an annual wellness exam, we perform essential screenings and provide guidance on various aspects of health, such as:

It's important to have a wellness exam every year, even if you're feeling healthy and don't have any specific concerns. Regular exams help ensure that any changes in your gynecologic health are identified and addressed early.
Depending on your age, overall health, and current screening guidelines, you may not need each part of the exam annually. However, we may suggest enhanced screening if you have risk factors, such as a family history of breast cancer, to ensure early detection and prevention.
When to Begin Annual Gynecological Exams
We recommend starting routine gynecological exams no later than age 21. Routine gynecological exams should be done once a year. Pap smears, which screen for cervical cancer, are no longer performed at each routine wellness exam. Though you may not need a Pap smear every year, you do need a wellness exam every year.
We would be happy to see you for an appointment before age 21 if you have any concerns about contraception, sexually transmitted infections, menstrual problems, or your general health. At Takoma Park Gynecology, we see patients from as young as 8 years old to well over 80 years old.

Gynecological Wellness Exam v/s Annual Physical Exam
A gynecologic wellness exam focuses on the prevention and early detection of various conditions (including cancer) of the breasts and sexual/reproductive organs. Concerns about contraception, reproductive life planning, and sexual health are often discussed during these visits.
An annual comprehensive physical exam typically focuses on general health and the prevention of chronic diseases like cancer, diabetes, and heart disease.
Aspects of a gynecologic wellness exam and an annual physical exam by your primary care provider may overlap. For example, vital signs are checked at both visits. At both visits, personal safety and mental health are assessed.
While both types of exams are important for your overall well-being, a gynecological exam provides specialized attention to your reproductive and sexual health.
We may provide one or both of these exams as needed to provide comprehensive care that addresses gynecological health and overall well-being in one visit.
What to Expect at the Annual Wellness Exam
At Takoma Park Gynecology, we start your annual wellness exam by reviewing your history. We will also record your vitals, such as blood pressure, heart rate, height, and weight.
Our providers will ask you about your:
- General health
- Pregnancy history
- Sexual activity status
- Contraception needs
- Past gynecological issues or surgeries
- Current symptoms or concerns
We may also conduct a physical exam, perform lab work, and order imaging such as a mammogram. This may include blood tests for blood sugar, cholesterol, and HIV. If applicable, we also discuss vaccinations appropriate for your age, such as the flu shot, Tdap, or HPV vaccine.
Annual Gynecological Exam By Age
Your health needs and the risks for certain conditions change as you age, which is why our gynecologic wellness exams are tailored differently for young adults, middle-aged women, and those in later stages of life.
- Teenagers (13-18): We meet you at this age to establish a relationship and talk about menstrual health. A pelvic exam is not always necessary at this age. The focus is more on education about reproductive health and preventing STIs.
- Young adults (18-35): For young women, the exam includes a pelvic exam and possibly a Pap smear based on age, as well as STI screening. Adults over 25 should also have an annual breast exam. Birth control options, sexual health, STI prevention, and reproductive life planning are also discussed.
- Middle-aged women (35-50): As women transition through their 30s and 40s, the focus shifts to monitoring reproductive changes, such as early signs of gynecologic conditions like fibroids or ovarian cysts, fertility concerns, and menopause.
- Postmenopausal women (50+): Postmenopausal women benefit from regular wellness exams to assess symptoms related to menopause, monitor bone density, and ensure overall health.
Tests & Exams Included in an Annual Wellness Exam
Your annual wellness exam includes a series of key tests and screenings to assess your reproductive health:

- Pap smear: This test screens for cervical cancer by detecting abnormal cells in the cervix. Depending on your age and history, it is typically performed every 3-5 years.
- HPV test: This test helps identify the presence of human papillomavirus, a leading cause of cervical cancer in women.
- Breast exam: It involves a manual check for any lumps, abnormalities, or changes in breast tissue.
- STI screening: Based on your age, sexual activity, and lifestyle, we may recommend testing for sexually transmitted infections (STIs).

Preparing for Your Gynecological Wellness Exam
Preparing for your exam can help ensure accurate results and a stress-free experience.
When a Pap smear is to be performed, try to schedule your annual exam when you are not having your period. Additionally, it is helpful to avoid douching or using vaginal products (creams, tampons, etc.) for 48 hours before the exam. You should also refrain from sexual intercourse for 48 hours before your appointment.
Bring a list of medications you’re currently taking, including supplements. You should also note any symptoms or changes you’ve experienced, such as irregular periods, pain, or discomfort.
We encourage you to openly discuss any concerns or symptoms you may have so that we can provide you with the best possible care.
