Loop Electrosurgical Excision Procedure (LEEP)

Loop electrosurgical excision procedure (LEEP) uses a thin wire loop charged with an electric current to remove abnormal tissue from the cervix. It is also called loop electrosurgical excision or loop excision.

LEEP is commonly used to treat cervical dysplasia (abnormal cell growth on the surface of the cervix) and other precancerous conditions. By removing these abnormal cells, it helps reduce the risk of these cells developing into cervical cancer.

At Takoma Park Gynecology, we are dedicated to providing expert care in performing LEEP. Our skilled providers ensure that you receive personalized attention in a comfortable and supportive environment.

From your very first consultation to post-procedure follow-ups, we prioritize your well-being and make sure that your experience is as smooth and stress-free as possible.

Understanding LEEP

LEEP is recommended when abnormal or precancerous cells are detected on the cervix, often after a Pap test or colposcopy. This procedure is used to help in the diagnosis or treatment of:

  • Cervical dysplasia
  • Precancerous lesions caused by HPV
  • Suspicious or abnormal biopsy results
  • Polyps (benign growths)
  • Genital warts

By treating these conditions early, LEEP serves as an effective preventive measure for the potential progression of cervical cancer.

Preparing for a LEEP

Before your appointment for a LEEP, we recommend the following guidelines to ensure a successful procedure.

  • Discuss any medications or supplements you're taking with our provider.
  • Avoid tampons and sexual intercourse for 24 hours before the procedure.
  • Bring an extra sanitary pad to wear after the procedure to manage any vaginal discharge.
  • Discontinue any medications that affect blood clotting after discussing it with your provider.
  • Inform us if you're pregnant or have any allergies.
  • Take a pain reliever about 30 minutes before the procedure.
  • Wear loose, comfortable clothes on the day of the procedure.
  • Arrange for transportation to drive you home after the procedure.

Please reach out to us if you have any questions or concerns before the procedure.

What to Expect During LEEP

LEEP is a safe and effective procedure when performed under expert supervision. Additionally, we can provide nitrous oxide for pain management during the procedure, helping you feel more at ease and enhancing your overall experience with compassionate, patient-centered care.

Here’s what you can expect with a LEEP.

Before the Procedure

Once you arrive at the clinic, we will brief you about the procedure and then ask you to empty your bladder. Once you are comfortably positioned in the exam chair or table, your provider will begin with a routine pelvic exam.

A grounding pad will be placed on your thigh to protect you from the electrical current used during the procedure, and local anesthesia is applied to numb your cervix.

During the Procedure

Once the anesthetic has taken effect, the provider will insert a speculum into your vagina to gently widen it and provide a clear view of the cervix. Next, a diluted solution of acetic acid or Lugol’s solution (iodine) is applied to your cervix to identify the abnormal cells.

A colposcope is then positioned near the vaginal opening to provide a magnified view of the cervix.

A thin wire loop, charged with electricity, is then passed through the speculum and positioned over the cervix. As the loop moves across your cervix, it takes away a thin layer of surface tissue, effectively removing the abnormal cells.

In some cases, a second loop may be used to access a deeper layer of tissue. You will be asked to remain perfectly still while the loop is being used to remove the abnormal tissue.

Once all identified cells are cleared, we apply a medicated paste to the treated area. The removed tissue is then sent to a lab for further testing to check for abnormal or cancerous cells.

The LEEP procedure takes around 20 minutes and has an excellent success rate.

After the Procedure

Once a LEEP is complete, we place you under observation for some time to confirm your well-being before discharge. We also provide you with detailed aftercare instructions and prescriptions for any necessary medications to prevent infection.

Recovery & Follow-Up After LEEP

Recovery from a LEEP is generally quick. You can return to normal activities within a day or two, although it’s advised that you avoid tampon use, strenuous activity, sexual intercourse, and douching for at least 2-4 weeks post-procedure.

Common post-procedure symptoms include:

  • Light vaginal bleeding
  • Mild cramping
  • Watery discharge

These symptoms are typical and should subside within a few days. However, if these symptoms persist, please contact our office for a consultation.

Please also remember to attend your follow-up appointments so our provider can confirm proper healing and assess you for any recurrence of abnormal cells.

You may need another LEEP if all abnormal cells are not removed fully in the first session or if abnormal cells reappear. The risk of recurrence is higher if you're infected with a high-risk strain of HPV. As a preventive step, we may also schedule a follow-up Pap test in 6-12 months.

Frequently Asked Questions

Is LEEP safe?

Yes, LEEP is considered a very safe procedure. It is minimally invasive and can typically be performed without the need for general anesthesia. As with any medical procedure, there are some risks, such as infection or bleeding, but complications are rare.

How do I manage discomfort after LEEP?

Mild cramping and light bleeding are common after LEEP. You can manage discomfort by taking non-aspirin pain relievers, such as ibuprofen or acetaminophen. Be sure to follow your provider’s recommendations for any additional medications or care instructions.

How long does it take to recover from LEEP?

Complete healing after LEEP typically takes around four to six weeks. During this time, your cervix will regenerate healthy tissue to replace the abnormal cells removed during the procedure.

When can I return to work or school?

Most patients can return to work or school within one to two days after their LEEP procedure, depending on how they feel. It's best to avoid any strenuous activities or heavy lifting for at least a week.

Does LEEP cure HPV?

No, LEEP does not cure HPV. However, it removes abnormal cells caused by the HPV infection, reducing the risk of them developing into cervical cancer. Your provider may suggest more frequent Pap smears or HPV tests to monitor for any new abnormal cells.

Is LEEP considered surgery?

Yes, LEEP is categorized as a surgical procedure, but it is minimally invasive and involves minimal downtime. Instead of traditional surgical instruments, LEEP uses a thin wire loop heated by electrical energy to remove abnormal tissue.

Will my cervix grow back after LEEP?

Yes, after LEEP, the cervix regenerates healthy tissue. Full healing typically takes four to six weeks, during which time the removed tissue is replaced by new, healthy tissue.
